How will I know what time to be ready for my ride?
Modivcare will tell you what time to be ready. If the pick-up time changes, we will call you.

What are the requirements to receive non-emergency medical transportation?
Transportation is for Healthy Connections Medicaid members who need to see a doctor, go to other medical visits or go to the drug store. The Member's medical condition should not be an emergency.

What do I need to have when I call for a ride?
- Medicaid ID number
- Pick-up address and phone number
- Appointment date and time
- Doctor's name
- Doctor's address and phone number

What if I need a ride and I am not able to call 3 business days before?
Urgent or same day rides may be scheduled through Modivcare. Rides are for trips when a doctor says you have to be seen the same day or the next day. Modivcare will call the doctor's office to make sure urgent or same day appointments are necessary.

What is not an Urgent Care trip?
Urgent trips are not the same as an emergency trip. In case of emergency dial 911. Urgent care trips do not include routine doctor's appointments when the patient could give a 3 business day notice to request a ride.

What type of transportation will I get?
Modivcare will ask if you can walk or if you require the use of a wheelchair and will then provide the best type of transportation for your needs.

When should I call for a ride?
Please call for a ride at least 3 business days before your scheduled appointment. Urgent trips may be accepted with less than a 3 day notice.

Can I schedule a ride using the internet?
Yes! You can sign up for an account by going to the member web site at https://member.modivcare.com/. The member website is secure and can be access 24 hours a day 7 days a week.

How do I get a ride for repeat appointments like dialysis?
Ask your medical provider to help schedule rides for you.

How do I report a complaint?
You can call any "Where's My Ride" number or any reservation number to report a complaint. Or you can file a complaint using the internet here.

How may I obtain this information in Spanish?

What if the ride is late or I have other problems with transportation?
If your ride is 15 minutes past pick up time, please call our "Where's My Ride" line.

What should I do if I don't need my ride or if I need to change my appointment?
Call Modivcare to cancel your ride as soon as you know you don't need it.

Call Modivcare to report the problem and file a complaint.

Modivcare will schedule a transportation provider that will meet your needs.
